SPA Questionnaires
The SPA core questionnaire reflects generally accepted standards for health care services, including UNICEF immunization guidelines and standards set by the Safe Motherhood Initiative. However, local working groups and experts in each service area adapt the SPA questionnaire to the information and health needs of each country. For example, the Kenya SPA included a review of childbirth records to collect information on specific delivery practices

SPA Audits
Audits collect information about the facility infrastructure, available equipment, drugs, pharmacy, and lab services, as well as record-keeping, management, and counseling. The audits provided here are the audits used in the 2007 Uganda SPA Survey.
SPA Observation Protocols and Exit Interview Questionnaires
Using the observation protocol tool, interviewers observe sick child, antenatal care, family planning, and STI consultations. Interviewers assess how well service providers adhere to national and international standards of care. Exit Interview Questionnaires are administered to clients after the observed consultations to assess client’s understanding and recall of provider instructions and other information, and the client’s perception of the service delivery environment. The observation protocols and exit interview questionnaires provided here are those used in the 2007 Uganda SPA Survey.
SPA Health Worker/Provider Interview
The Health Worker/Provider Interview collects information from providers on pre-service and in-service training, supervision, and attitudes about their work environment. The health worker interview provided here is the questionnaire used in the 2007 Uganda SPA Survey.
